I think Spacetrack has some history of using decay notices for objects that have actually left earth orbit in the outwards direction.

Space Track.org now shows LADEE as decayed on 7 October 2013.
 
Does this mean the orbit insertion failed and that the spacecraft impacted on the Moon? Of course NASA is as quiet as a doornail as a result of the irresponsible behaviour of Congress.
